  1. Referral and information – We agree on what needs to be assessed and I will gather background details about the person being assessed and the decision to be made.

  2. Assessment meeting – We agree a convenient time and place for the assessment (at home, in a care setting, etc). We have a conversation to talk through what decisions are being made and the persons understanding.

  3. Report preparation – I write a clear court ready report explaining my findings and opinion.

  4. Feedback and next steps – The report is shared with you and your representative e.g. your solicitor, and I can answer any questions.
